Adjunct A/Prof Chong Phui-Nah has been Chief Executive Officer, National Healthcare Group Polyclinics (NHGP) and Primary Care in Singapore since 2014. She is also Senior Consultant Family Physician, Fellow of College of Family Physicians, Singapore and member of the Academy of Medicine, Singapore. In addition to her administrative role, she also helms Family Medicine Board which oversees and sets the direction for clinical services and patient care, education, and research as well as Quality Council in NHGP. She continues to practise at her Family Physician Clinic once a week.

She is a strong advocate for patient-centred care, paving the way for innovations. NHGP was awarded MOH Excellence Champions Medal at MOH National Healthcare Innovation & Productivity 2019 for NHGP’s Teamlet Care delivery model for chronic disease management and Best Practice Medal for Care Redesign: Teleconsultation Services for the care of Chronic Conditions at MOH National Healthcare Innovation & Productivity 2021.
